Carol Ann (Kozaka) Kozik, 78, of Chicopee, passed away on Saturday, October 9, 2021 at Mercy Medical Center in Springfield. She was a retired medical assistant for Drs. Burger and Weatherbee. Carol was born on October 15, 1942 in Springfield and was the daughter of the late Theodore J. and Anna M. (Swol) Kozaka. Carol was a graduate of Chicopee High School, Class of 1960. She was an avid Red Sox fan and loved all her grand dogs. Besides her parents, she is predeceased by her husband, Chester W. Kozik. Carol is survived by her daughter, Ann Marie Liswell and her husband, Robert of Chicopee, her grandson, Colby Liswell and her brother, Theodore Kozaka of Cromwell, CT. All services will be private.
